PERMIT SUBMITTAL REQUIREMENTS

The following documents must be provided in order to successfully complete this form. Submittals must include appropriate fire permit application and requirements (no exceptions).

REQUIREMENTS FOR REQUESTING AN EMERGENCY PERMIT

Emergency permits are issued for existing systems that are in need of repair.

Once you have completed the form, you may start working to get the system repaired to proper working order. After the emergency permitting request is received, you will be contacted during regular business hours with the next steps.

Please Note: maintenance work is not considered emergency work.
